**Purpose of Amendment 0004: The purpose of this amendment is to respond to the RFIs and incorporate Exhibit N - 2023 Bituminous Specifications. The SF30 which addresses the RFIs is issued as actual Amendment 0003. As a result of the RFIs the following sections have been revised: B-Supplies or Services and Price/Cost, C-Description/Specifications/Work Statement, E-Inspection and Acceptance, L-Instructions, Conditions, And Notices to Offerors, M-Evaluation Factors for Award and Revised, and Exhibit M - Exhibit Line Item Pricing Paving IDIQ. The following are being provided in word format as applicable: Revised Exhibit A - Offeror Team Composition and Management, Exhibit B - Experience ? Offeror, Exhibit C - Past Performance Questionnaire (Form PPQ), Exhibit D Safety Data, Revised Exhibit E - Financial Questionnaire, and Exhibit F - Surety Form, Revised Exhibit I - Wage Determination AZ 20240042 (*PDF). The response date has been extended from April 26, 2024, to May 09, 2024 at 2:00 PM AZ Time. See SF30 attachments for additional details.
