The Network Contracting Office 17 intends to award a single firm-fixed price base plus four option year indefinite-quantity indefinite-delivery contract in accordance with FAR Part 12, Acquisition of Commercial Items for the purchase of TAP Sleep Apnea Devices (see attached RFQ) for the Central Dental Lab at the Dallas VA Medical Center. This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. The solicitation number is 36C25718Q0302. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-92, effective December 19, 2016. The associated North American Industrial Classification System Code (NAICS) for this procurement is 339114 with a size standard of 750 employees.
