There is no page limit to this Attachment SERVICE DESCRIPTION, FUNDING AND CONTRACTING INFORMATION: PURPOSE AND OVERVIEW The Multnomah County Health Department (MCHD) is seeking proposers to provide telepsychiatry services (MD and PMHNP) on a temporary, on call basis and/ or permanent placement to staff any of the Health Department’s health care facilities on an as needed basis, including Multnomah County Corrections Health (MCCH), Multnomah County’s Community Health Center (ICS), and County field- based services, school based clinics, Corrections Health provides medical and mental health services to individuals in legal custody at the Multnomah County Detention Center, the Multnomah County inverness Jail, and the Donald E. Long Juvenile Detention Centre. Staffing services are for telepsychiatry, only. The following definitions are provided for the purpose of this RFP: TELEPSYCHIATRY 100% of services will be provided via telehealth (phone or video). ON-CALL On call Providers are Health Care Providers that can be Pre-Scheduled or requested within a specific time period. LOCUM TENENS A Provider who fills in temporary for another employed provider. We have a current need for Locum providers who can work a specific length of time at a specific clinic on a consistent basis. PERMANENT PLACEMENT A Candidate recruited by Proposers to fill a regular, ongoing position within MCHD. Candidates will need to be screened and credentialed through the County, but may provide services through agency during transition into County employment. Federally Qualified Health Center (FQHC): if licensed independent practitioners (LIPs), are coming to MCHD as in health center employees, individual contractors, or volunteers, then we need to look at the type of credentialing they will need to complete before they see clients (Examples: Internal credentialing based on HRSA/Bureau of Primary Health Care/Chapter 5 clinical staffing credentialing requirements, health plans network, Participation/provider enrollment, DMAP/ Medicaid and Medicare enrollment for LIPs only). Permanent Placement may be requested for positions that MCHD has a difficult time filling and in turn may require national recruiting, such as MDs/NPs. Description https://solutions.sciquest.com/apps/Router/SupplierLogin?CustOrg=Multnomah INTRODUCTION AND PROGRAM HISTORY The MCHD is Oregon’s largest Public Health Department and has provided health services to low-income county residents since 1860. As part of its array of services, MCHD operates a vast array of health centers including but not limited to seven primary care clinics, seven dental clinics, and a specialty HIV clinic, seven in- house pharmacies and three corrections Health clinics within Multnomah County’s correctional facilities. Currently, MCHD provides services to over 60,000 clients in its primary care clinics, and over 1,300 inmates at its maximum security, medium security and juvenile facilities. MCHD health care services include, medical, dental and prenatal care, disease management for chronic illnesses, behavioral health services, corrections health, family planning, pharmacy, school-based health services targeted for children and adolescents, specialized care for HIV- positive individuals and preventive services such as well- child health care, immunizations and nutrition services. By integrating these health services, MCHD provided clients with continuity of care while improving operational efficiency and maximizing health improvements for special populations. MCCH currently oversees the mental and physical well-being of approximately 1,100 inmates at its maximum security, medium and juvenile facilities with clinical services being delivered 24 hours a day, 7 days a week. MCCH employs a broad array of clinical staff as part of an interdisciplinary care team, including MD’s, NPs, Community Health Nurses (Registered Nurses), Qualified Mental Health Professionals, and CNAs. GOALS, VALUES AND OTHER IMPORTANT CONSIDERATIONS It is imperative to the county that the contractor encompasses the following values in its operations and/ or in identifying and placing providers in any MCHD site:  Believe that health is a “state of complete physical, mental, and social well-being, not merely the absence of disease or infirmity.” (World Health Organization, 1978).  Honor the diversity of the individuals and communities we serve and value their differing approaches to health and well-being.  Value a diverse pool of providers and believe providers should be selected with care, treated with respect, held accountable for their performance and encouraged in personal growth.  Believe in continuously improving the quality of work.  Believe in balancing scientific knowledge and practical experience with the wisdom and beliefs of those we serve to improve the health of our communities.  Emphasize prevention, health promotion and early intervention. For work done in MCCH, there are additional considerations that relate to the provision of healthcare in a jail environment, MCCH provides services which promote and protect the health of adults and juveniles entrusted to their care. MCCH, with their community partners, including the MCSO, protects the rights of their clientele with essential health care including medical, mental health and dental services. It is expected that the contractor will work with MCCH and the MCSO to provide quality health services care in a restricted setting. The Contractor will be working with a diverse group of clients coming from various backgrounds who have a variety of mental health and medical issues. It is imperative that every patient is treated with respect making sure clients are not unnecessary agitated is part of that expectation. The Contractor will be expected to work with clients with a wide range of criminal charges and it is important that each client, without exception, will be treated with professional courtesy and dignity. In addition, the contractor should understand that at all times while in the MCCH jail facilities, they shall obey all instructions from MCSO staff. All Contractor employees, including clinical staff and support staff (e.g. recruiters doing site visits, management) will be required to undergo an MCSO criminal background check that will be provided free of charge and coordinated with MCCH. The Contractor should also understand that access to the facility needs to be pre-arranged and cleared through the MCSO office. Contractor employees will be required to undergo Safety and Security Training, one time only, provided by the Multnomah County Sheriff’s Office. In addition, Contractor employees will need to participate in a 3hr online training in PREA (Prison Rape Elimination Act). Both training will be provided by corrections Health and will be free for contractor employee. The Contractor should understand that MCSO security personnel will be required to supervise and direct clinical care. Where deemed appropriate by MCSO, security concerns may require the use of some physical restraints (handcuff or leg iron, or other device) during a clinical encounter. Security staff will transport and supervise the detainee. Every effort should be made to preserve HIPAA compliances and to ensure that the client receive appropriate clinical care, however security will remain an essential concern. GOALS The Contractor must meet the following goals, which are described in detail in the Scope of Work section in this RFP.  Timeliness in responding to staff order  Candidate availability is responsive to needs of 24/7 operations and can meet scheduling requirements across different shifts/days of work  Attract quality candidates, including those with expertise working in the FQHC settings  Demonstrated effort in placing diverse and bi-lingual candidates  Quick remediation of personnel issues. TARGETED POPULATION SERVED The following narrative provides a brief overview of the target populations within each of the main service areas under the scope of this solicitation. Community Health Center Clinics (ICS): MCHD’s seven primary care are the largest service providers for Medicare and Medicaid patients in Multnomah County. Currently, MCHD provides services including pharmacy services to over 60,000 clients in its primary care clinics. MCHD also provides services within student health centers and dental clinics across the county. Our patients come from very diverse backgrounds and socio-economic levels. Multnomah County Corrections Health (MCCH): MCCH provides the highest degree of level of care for the inmates located within the criminal justice system in a cost effective manner. There are three corrections facilities. Multnomah County Detention Center (MCDC), Multnomah County inverness jail (MCIJ) and Juvenile Detention Hall (JDH). MCDC: is a maximum security jail that houses male and Female County, state and federal prisoners located in downtown Portland with 448 available jail beds, MCDC is the intake (booking facility) for all Multnomah County. MCDC operates under comprehensive written policies and procedures governing medical and mental health treatment. MCDC houses the highest acuity medical and mental health inmates. MCIJ: is a medium security adult male and female correction facility located in Northeast Portland. It houses 862 available jail beds where MCCH medical staff also work in conjunction with Multnomah County Sheriff’s Corrections officers to ensure the community standard of health inmates. JDH: is a juvenile detention center that houses 80 beds. The age of the inmates range from 5-18 years of age. MCCH works in conjunction with the Department of Community Justice (DCJ) staff to maintain a community standard of health care for the juveniles during their incarceration. GEOGRAPHICAL BOARDERS/LIMITATIONS & SERVICE AREAS Services will be performed at the facilities listed below:  Inverness Jail: 11540 NE Inverness Dr. Portland, OR 97220  Juvenile Detention Centre: 1401 NE 68th Ave, Portland, OR 97213  Health Services Center: 426 SW Stark Street, 5th floor, Portland, OR,97213  Any/all Community Health Clinics, Dental Clinics and Student Health Centers within Multnomah County Locations may be added or deleted over the course of the contract period. FUNDING The funding source of this contract is based upon reimbursement from insurance payors and the ICS enterprise fund. Services will be provided on an as needed basis. It is estimated that up to $ 350,000 of services will be purchased each contract year. Funding of the work described in this RFP is not guaranteed. Fluctuations in funding year to year should be expected. The County cannot assure that any particular level of work will be assigned and the contract will permit the County to add or remove work as necessary depending on availability of funding. MATCH REQUIREMENTS Not applicable to this Solicitation. SCOPE OF SERVICES Contractors are required to provide licensed health care professionals to provide direct and indirect telepsychiatry services and care to MCHD clients in primary care setting. The following sections provide detailed descriptions of the scope of work of the Contractor and the different types of healthcare professionals. Contractors’ responsibilities: For Scheduling of On-Call/Locum Licensed Providers.  To provide expertise in recruiting physician and nurse practitioner candidates who excel in telepsychiatry.  To provide project management supportive of developing a telepsychiatry service.  To initiate placement of a candidate, Contractor will furnish CV/ Resume and assist in scheduling short informational interview with candidates per County’s request.  Provide copies of current OR licensure, certifications, DEA, upon request for all contract providers that may be assigned work at any Health Department facility.  Notify County of inability to fill a request with a current, prequalified candidate.  Maintain client confidentiality in compliance with HIPAA provisions.  Services will be available Monday-Friday, 8am- 5pm; some evening and weekend hours may be needed, but would be negotiated with the provider.  Contractor will be required to provide all documentation required by agency for credentialing and submit prior to on-site start to Health Department staff. Credentialing process can take up to 90 days for completion.  Contractor is responsible for maintaining and providing proof of medical malpractice coverage. Contractor responsibilities: for permanent placement requests.  To initiate placement of a candidate, Contractor will furnish CV/ Resume and assist in scheduling short informational interview with candidates per County’s request.  Provide copies of current licensure, certifications, DEA, contract providers that may be assigned work at any Health Department facility.  Contractor will be given two weeks to produce at least one viable candidate that fulfills the minimum requirements set forth based on position.  Contractor will assist in scheduling site visits and interviews with Health Department staff and candidate.  All Providers assigned to a MCHD facility must pass a Multnomah County Sheriff’s Department background security check, including completion of the Authorization of Records Check form (See Attachment B in the Buyers Attachment Section of this sourcing event). This form states that the signer understands that a check of the criminal history records will be made before employment with MCCH. Their signature authorizes the background check and acknowledges that they are aware that a record of convictions or pending criminal action may exclude signer from employment with MCCH. It requests name and any other names used, current address, driver’s license, date of birth, hair color, eye color, height, weight, race and sex, as well as a copy of the applicant’s government -issued ID/driver’s license.  Proof of fingerprinting, as requested by the County. Any Costs associated with fingerprinting will also be the sole responsibility of the Contractor.  All providers assigned to MCCH must comply with Multnomah County Corrections Health Dress Code (See Attachment C at the Buyers Attachment Section of this sourcing event).  All providers assigned to MCCH will be required to get an MCSO Badge following successful completion of the background check.  All providers assigned to MCCH will be able to fulfill job requirements after County provided training, training duration will vary by professional type( e.g., Nursing staff may get a more extensive training than CAN staff). Training will be a minimum of 40- hour training( in five (5) eight(8) hours shifts) on –site orientation, including participation in Multnomah County HIPAA training, Bloodborne Pathogen Training, PREA Training, HER Training, MCSO jail Safety Training, and review/attestation of County personnel rules/e-policies as directed by County management.  Contractor will be required to provide all documentation required by agency for credentialing and submit prior to on-site start to Health Department staff. Multnomah County credentialing process can take up to 90 days for completion.  Contractor is responsible for maintaining and providing proof of medical malpractice coverage. Personnel Requirements All providers assigned shall provide the following:  Current licensure as required by the State of Oregon  Current DEA license ( as required for MDs, NPs)  Proof of certification (board certified, ANCC, etc.) Personnel Limitations  No J-1 visa applicants or HB-1 opportunities for telepsychiatry providers.  ICS would consider J-1 visa applicants or HB-1 opportunities for psychiatrists providing 100% in person care with linguistic and cultural knowledge, skills, and abilities aligned with ICS patient demographics. FISCAL REQUIREMENTS AND REPORTING Reports will be required monthly clearly documenting each name of provider, role, hours worked, rate of pay, facility and will include documentation of authorization of site designee. The County encourages the use of email report transmission to promote sustainability. At time of contact negotiations, details on required formats, timeliness and where and how to submit reports will be determined. PERFORMANCE MEASURES/PERFORMANCE CONTRACTING The awarded contractor will be measured on their performance by the following metrics:  Experience with placing telepsychiatry providers in the ambulatory care setting.  Accurate submission of invoice within fourteen (14) days of end of service.  Timely submissions of reports required above.  Speed of fulfilling request: Contractor will have fourteen (14) business days to submit viable candidate.  Candidate rejection shall be no more than 50%.  Fulfilment of placement requests according to the timeliness outlined above.  No substantiated complaints regarding professional conduct or client care.  Responsiveness to staffing or invoicing requests within 2 business days regarding current placement requests. CONTRACT NEGOTIATION Once selected in the RFP process, The County will initiate contract negotiations with the Proposers who score a minimum of 70% in the evaluation process. Multnomah County may, at its option, elect to negotiate general contract terms and conditions, services, pricing, implementation schedules, and such other terms as the County determines are in the County’s best interest. If negotiations fail to result in a contract, the County reserves the right to terminate the negotiations and initiate contract negotiations with another qualified Proposers. This process may continue until a contract agreement is reached. CONTRACT TERM This RFP will be potentially result in the award of multiple Fixed Term contacts for the period beginning October 1, 2022. The Department reserves the right to withdraw this RFP at any time, re-issue a subsequent RFP or terminate the resulting contracts, if any, at any time. COMPENSATION AND METHOD OF PAYMENT The contractor shall provide the County with an itemized invoices of services rendered once a month. The invoice shall include the invoice number, date and cost of the services. The County will reimburse the Contractor within 30 days of submission of invoice each month. Contractor will be required to sign up for the County’s electronic funds transfer (Automated Clearing House) or by credit via ePayables to receive payment of services. INSURANCE REQUIREMENTS The table below states the minimum insurance required of a Contractor to provide this service. Additional insurance coverage may be required depending on the key features of service delivery chosen by the Contractor. Final Insurance requirements will be subject to negotiation between, and mutual agreement of, the parties prior to contract execution and recorded in Exhibit 2 of the contract. The minimum insurance coverage stated below supersedes requirements listed in Exhibit 2 of the Sample Contract where they are different. Failure to provide any of the required documents or meet any of the below requirements shall result in rejection of the proposal.  The Proposal response must be received by Multnomah County Purchasing no later than 4:00 P.M. local Portland time on the proposal submission deadline. Proposer Representations and Certifications  The Proposer must certify that they agree to the Proposers Representation and Certification terms in the Pre-requisite Section of the Sourcing Event. At the time of Contracting, Proposers must meet the following minimum requirements. Failure to provide any of the required documents or meet any of the below requirements shall result in a determination of non-responsiveness and may result in award to the next highest proposer.  Proposers must be legal entities, currently registered to do business in the State of Oregon (per ORS 60.701).  Proposers must submit verification that all insurance requirements are met. Proposers must have a completed Pre-Award Risk Assessment if federal funds are used for this Sourcing Event. (See Procedural Instructions in the Buyer Attachments Section of this Sourcing Event). (10% of the Programmatic questions). 2.1.5 2.1.5: Once a match has been made, describe the process by which MCHD would work with proposer in facilitating onboarding of a telepsychiatry provider, including experience working with practice managers and health information services trainers. Evaluation Criteria: The Proposer – Has the experience of matching telepsychiatry providers in the ambulatory care/FQHC setting. Has experience in arranging interviews with multiple stakeholders. Has experience facilitating credentialing of licensed independent providers. Has described a well-positioned process for co-facilitating telepsychiatry onboarding. If so please list the types of courses offered as well as how access is provided to your candidates. Provide a Rate Sheet listing all the hourly rates and fees associated with the services outlined in Scope of Services by provider types(s). Identify on the Rate Sheet whether costs/fees are hourly, monthly, annually, etc. and if there are any relevant differentials that could apply (e.g., shift differential, language, holiday); include rates for on call/locum and permanent placements where applicable.
