RFA 5050 - Comprehensive COVID-19 Testing Services

1.1 INTRODUCTION
The State of Oregon, Oregon Health Authority (OHA) is issuing this Request for Applications ("RFA") pursuant to Governor Brown's declaration of emergency to respond to the Novel Coronavirus Disease 2019 ("COVID-19") and the need to procure services for testing for COVID-19 for residential care facilities throughout the State of Oregon. Applicants need only respond once for consideration under this RFA, but are encouraged to modify and update their Applications, if necessary and applicable pursuant to Addenda to this RFA, to address new Application requirements for providing services in additional areas of the state or additional types of services included in one or more RFA Addenda.
OHA anticipates awarding multiple Contracts to

qualified Applicants from this RFA. The initial term of the Contract(s) will be determined at the time of Contract negotiation and OHA anticipates the award of multiple Projects under the initial Contracts awarded under this RFA.
OHA will have the option to renew and extend the Contract for additional years and add additional funding, up to a maximum term of seven years and to increase the total not-to-exceed consideration, amend the specific services to be performed and, or alternatively, the rate of payment, by written amendment signed by both parties.
OHA anticipates multiple projects under the Contracts awarded from this RFA. However, a Contract award under this RFA is not a guarantee that the Contractor will be selected to perform services for any project for OHA.
Rather, all Work by Contractor(s) under the Contracts awarded from this RFA shall be performed only upon OHA's written request to OHA-selected Contractors per Item 1) below, and following OHA's written approval of the project plan for each project as described in item 2) below.
