COMBINED SYNOPSIS/SOLICITATION Indoor Air Quality Testing Services at Michael E. DeBakey Veterans Affairs Medical Center in Houston, Texas(I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.(II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number: VA256-16-R-1024.(III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-87.(IV) This solicitation is being issued as a Total Service Disabled Veterans Owned Small Business Set-Aside. The associated NAICS code is 541620, Environmental Consulting Services. The small
business size standard is $7.0 Million. (V) This combined solicitation/synopsis is for purchase of the following commercial services: Contractor shall furnish all labor, equipment, skilled and certified personnel to conduct Indoor Air Quality (IAQ) investigations at the Michael E. DeBakey Veterans Affairs Medical Center main campus located at 2002 Holcombe Blvd, Building 100. Houston, Texas 77030.(VI) Description of requirements is as follows: Contractor shall furnish all labor, equipment, skilled and certified personnel to conduct an Indoor Air Quality (IAQ) investigation at the Michael E. DeBakey Veterans Affairs Medical Center main campus located at 2002 Holcombe Blvd Houston, Texas 77030. The specific areas are the 2nd floor area H and the PET/CT area located on the 1st floor. This solicitation assumes an investigation of the specific areas as a response to IAQ complaints. The 2nd floor area H is approximately 20,000 sq. ft. and consists of about 50 rooms and 3 major corridors and is served by 1 Air Handling Unit and 2 return fans. The PET/CT area is approximately 1200 sq. ft. and is at the end run of the areas served by Air Handling Unit 1L. See attached drawings. Most work will be performed during VA normal working hours of 8 am to 4:30 pm Monday-Friday excluding federal Holidays. Some of the work, such as infrared scanning, may need to be performed after hours. Interested parties see attached Statement of Work which applies to the one time performance of Indoor Air Quality Testing, a Tentative award date of 08/01//2016 and completion date before 09/30/2016. DeBakey VAMC, 2002 Holcombe Blvd, Houston, Texas 77030Attention: Offerors are advised to read all submittal instructions, Statement of Work and Evaluation Factors thoroughly before submitting a proposal. Failure to follow the detailed instructions could render a proposal unacceptable. All proposals must contain a detailed explanation to how each task listed in the SOW will be accomplished by the offeror. Period of performance The period of performance of this contract is tentatively set for August 01, 2016 until Task is complete IAW Statement of work, not to exceed September 30, 2016.
