Statement of Work (SOW) Maintenance Service Contract (2) Strippit Press Brake Machines 1.0 Scope: The contractor shall provide all labor, supervision, routine preventive maintenance items (sealants, lubricants, oils, etc.), and any other tools and equipment necessary to service two (2) Strippit CNC press brake machines. The machine plant account numbers are: 65586-058599 and 65886-067119 2.0 Levels of Service: 2.1 Preventive Maintenance- The contractor shall perform preventive/scheduled maintenance, calibration, and system inspection in accordance with the Original Equipment Manufacturer (OEM) manual. The contractor shall perform scheduled maintenance two (2) times per year (every six months). 2.2 Emergency Maintenance- Emergency maintenance consists of services outside of scheduled preventive maintenance services for broken and/or unserviceable components that will stop production of the press brake machines. The contractor shall perform emergency maintenance repairs on all
systems/components of the machines that breakdown and/or become unserviceable outside of normal preventive maintenance schedules. It is anticipated that 0-2 emergency repairs will be required during each performance period. The contractor shall be on site to provide emergency maintenance within three (3) business days of notification by the Government technical representative. 2.3 Replacement Parts- If the contractor identifies parts that require replacement (other than those parts included in routine preventive maintenance) during scheduled or emergency maintenance inspections, the contractor shall submit a written listing of these parts along with associated costs including labor to the Government technical representative immediately following inspection. The Government technical representative will verify the contractorâ€TMs proposed work, materials, and pricing for accuracy and adequacy before submitting the contractorâ€TMs quote to the Contracting Officer for authorization to replace parts. Any parts replaced (other than those included in routine preventive maintenance) without verification from the Government technical representative and authorization from the Contracting Officer shall be at the contractorâ€TMs expense with no reimbursement by the Government. If repair parts have to be ordered, the contractor may leave the site and return within two (2) business days after the parts arrive to complete services. 3.0 Disposal: The contractor shall dispose of any debris and/or leftover materials upon completion of any services outlined in this SOW. 3.1 Safety Data Sheets- The contractor shall provide Material Safety Data Sheets (MSDSâ€TM) for all measuring equipment, solvents, grease sealants, and lubricants used to provide any service outlined in the SOW. 4.0 Business Hours: Services shall be performed during standard Fleet Readiness Center Southeast (FRCSE) working hours. Refer to paragraph 2.1.10 of the attached FRCSE Work Restrictions Excerpt. The contractor shall contact the Government technical representative to arrange timeframes for conducting services. Note: Travel costs shall be included the price of maintenance and shall not be billed separately. 5.0 Place of Performance: Services shall be conducted at the FRCSE, Naval Air Station (NAS), Bldg. 101, Jacksonville, FL 32212.
