You are adept at coordinating between various departments and social service agencies. An example of your duties include: consulting on client care and offering solutions/resources; department collaboration for service referrals; empowering and educating clients and their families; conducting home visits and phone calls following best practices; record maintenance; teaching preventive health and health behaviors; serving as a liaison with wider agencies to advocate for resident needs; and collecting data for quality improvement projects.
