REQUEST FOR QUOTE (RFQ) 36C2622Q0058 VHA Greater Los Angeles Healthcare System: Safety Shoes COMBINED SYNOPSIS/SOLICITATION GENERAL INFORMATION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 - Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR 13 Simplified Acquisition Procedures; and supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2019-05, dated August 13, 2019. The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office is issuing a Request For Quote (RFQ) #36C26220Q0058 for Small Business Set-Aside Competition with the size classification relative to NAICS codes 316210 or (related) that can supply the VA Greater Los Angeles Healthcare System with Safety Shoes. This is a brand name only requirement.

DESCRIPTION OF REQUIREMENT 1 SAFETY SHOES ANNUAL ALLOTMENT SAFETY SHOES FOR SCM/ENG/EMS/SAFETY STAFF CONTRACTOR WILL SERVICE THE VHAGLA HEALTHCARE CENTER WITH A MOBILE SHOE TRUCK AT SPECIFIED LOCATIONS TO ADDRESS BOTH THE WLA AND SEPULVEDA CAMPUSES.TRUCK MAY BE REQUIRED AT EACH LOCATION NO MORE THAN TWO (2) TIMES PER YEAR. EACH EMPLOYEE IS AUTHORIZED ONE PAIR OF EITHER SHOES OR BOOTS. Contract Title. Safety Shoe 2. Background. This is an annual requirement. 3. Scope. Service of Mobile Shoe Truck to provide safety shoes availability, two times within the period of performance. This support is needed to provide staff mandatory requirement of safety shoes on-site at WLA and Sepulveda locations. 4. General. The primary purpose of this service is to provide each authorized employee one pair of either shoes or boots. Shoes will be provided on day of fitting, except special orders which will be delivered within 3 days. 5. Specific Tasks. Service of Mobile Shoe Truck to provide on-site availability (approximately) February 2021 and August 2021 at VAGLA WLA and Sepulveda campus. Vendor will provide shoe fitting of each employee and shoes on the day of the fitting. Performance Monitoring. The point of contact will monitor service to ensure all necessary staff to receive safety shoes on days of visitation or receive voucher. 7. Security Requirements. Estimate period of performance shall be commencing 2/1/2021 and ending 8/31/2021. 13. Delivery Schedule. N/A 14. CONTRACTOR EMPLOYEES ROSTERS N/A. 15. IDENTIFICATION OF CONTRACTOR EMPLOYEES: a. The Contractor shall ensure that the employees assigned shall be cognizant of sexual harassment, drug-free workplace and other appropriate federal employment requirements. b. Contractor s Employees: Both parties agree that the contractor performing under the contract shall not be considered VA employees for any purpose and shall be considered employees of the contractor only. In addition, the COR shall act as the primary point of contact (POC) between the CO and the contractor. The representative shall have full authority to act on behalf of the contractor on all matters relating to daily operations of contract performance. An alternate POC shall also be designated. SALIENT CHARACTERISTICS 1. Must be steel toe or composite construction that meets the OSHA Safety Shoe requirement. 2. The outer construction can be leather or other soft material that meets OSHA standard. 3. Slip proof sole. 4. Vendor should have the ability to deliver (by post) and or accept store walk-in vouchers. 5. Vendor must have a mobile service that can deliver to campus for mass distribution of product. All offerors shall be an authorized distributor by the manufacturer and must be able to offer full manufacturer warranties for products delivered to the Government. All quotes will be evaluated based on Lowest Price Technically Acceptable (LPTA). Responses to this RFQ are due by January 22, 2021 Pacific Standard Time (PST).
