If the supplier chooses to submit the bid directly to purchasing in writing, attached bid form must be used. The bid must be signed in ink, sealed in a properly addressed envelope, and delivered to the Office of the City of Bossier City Purchasing Agent, 2nd Floor, Municipal Building, 620 Benton Road, Bossier City, LA 71111 by the "Due Date and Time." The "Bid Number", “The Bid Title”, and "Due Date" should appear on the outside of the envelope. On any bids submitted in the amount of $50,000 or more, the Contractor shall be licensed under Louisiana Revised Statutes 37:2150-2163 and show his license number on the bid and envelope in which it is submitted. 5. All prices and notations must be in ink or typewritten. Each item must be priced separately. Unit price shall be shown and a total price shall be entered for each item bid. Errors may be crossed out and corrections printed in ink or typewritten and initialed in ink by person signing bid. If there is an error in the extension, the unit price will govern. 6. Bids, modifications, or corrections received after the closing time on the "Due Date" will be considered late and will be considered non responsive. 7.
