Suicide Prevention

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included with this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.The United States Property and Fiscal Office for South Carolina, 9 National Guard Road, Columbia, South Carolina intends to award a firm fixed price contract for the South Carolina National Guard for Suicide Preventions training and training materials. The basis for award will be best value to the Government. All quotes are to be submitted in the following structure:CLIN 0001:  QTY of 30, Unit of Issue is Each - Online Suicide Prevention training that provides a 5 to 6 hours of online learning featuring information for faith based leaders.  The training needs to include scripture, theological perspectives, and have a moral basis for a life promoting approach to suicide

prevention. Contractor shall provide surveys that assess changes in participants’ knowledge, understanding, and attitudes towards suicide for the above training   CLIN 0002: QTY of 60, Unit of Issue is Each - Eight Hour/One day prepackaged trainings kits which consists of a workbook and character cards for Suicide prevention focusing on helping people with lingering thoughts of suicide. Additionally, at least nationwide up to worldwide certificates need to be provided.  Contractor shall provide surveys that assess changes in participants’ knowledge, understanding, and attitudes towards suicide for the above training.  Training will be conducted by Government personnel utilizing the training kit. Delivery of the kits included to Columbia, SC.CLIN 0003:  QTY of 121, Unit of Issue is Each - Four hour/half day prepackaged trainings kits which consists of a workbook and an intervention tool card for Suicide prevention focusing on breaking down the stigma associated with suicide and the community. Can be used with participants between the ages of 15 years and up. Additionally, at least nationwide up to worldwide certificates need to be provided. Training will be conducted by Government personnel utilizing the training kit. Contractor shall provide surveys that assess changes in participants’ knowledge, understanding, and attitudes towards suicide for the above training. Delivery of the kits included to Columbia, SC.CLIN 0004: QTY of 60, Unit of Issue is Each - Suicide prevention two-day prepackaged trainings kits which consists of a workbook, intervention tool card, national suicide lifeline card, reminder sticker, and a community resource sheet focusing on preventing suicide, recognizing signs, providing an intervention model, and must develop a safety plan to keep someone alive.  Additionally, at least nationwide up to worldwide certificates need to be provided. Training will be conducted by Government personnel utilizing the training kit. Contractor shall provide surveys that assess changes in participants’ knowledge, understanding, and attitudes towards suicide for the above training. Delivery of the kits included to Columbia, SC.CLIN 0005: QTY of 34,405, Unit of Issue is Each - Interactive pre-recorded 60 to 90 minutes of online suicide prevention training. Online training needs to teach trainees to recognize when someone is thinking about suicide and connect them to help and support. At least nationwide up to worldwide certificates need to be provided at the end of the training. Contractor shall provide surveys that assess changes in participants’ knowledge, understanding, and attitudes towards suicide for the above training.CLIN 0006: QTY of 1, Unit of Issue is Each - Evaluation Report of Surveys – From the data received in the surveys, Contractor shall provide a report summarizing the changes in participants’ knowledge, understanding, and attitudes towards suicide.  For the online training, the Contractor is required to issue, collect, and then supply an evaluation summary of the surveys. For the training being conducted by Government personnel utilizing the Contractors’ training kits, the Government will collect and then supply the Contractor the surveys and the Contractor shall provide an evaluation report of the surveys.The NAICS Code for this solicitation is 611430 with a size standard of $12 Million. The corresponding PSC is 6910. This procurement is set aside as a total small business set-aside. The Government will not consider partial submission. You must quote all CLINs in order to be considered for this procurement.Solicitation W912QG-21-R-0049 is issued as a Request for Quotes (RFQ) in accordance with FAR Parts 12 & 13. To be considered for award, the offeror must be registered in SAM.All offerors must include the following along with their offer: DUNS Number, CAGE Code and Federal Tax ID Number. All offerors must also sign and return the three attachments to this solicitation.
