Issuing agency:County of Santa Clara, CASee other Bids by this agency Issuing department:HEALTH AND HOSPITALSee other Bids by this department Bid Comments: The County of Santa Clara Health System (“County” and/or “Health System”) seeks proposals from qualified and experienced vendors, hereafter (“Offerors”) to provide a consolidated system of care for patients entering the County’s Health System emergency departments (“ED(s)”) currently located at SCVMC, OCH and SLRH, including patients who are treated and released or awaiting acute or observation admission. In addition to the EDs, the Offeror will staff and manage the County based Express Care Clinic at SCVMC (“EXC”). Last, Offeror shall provide Physicians and advanced practice provider (“APP”), when required by the County, who shall provide on-site urgent care staffing services to County patients at the County’s urgent care facilities, as the same may change from time to time. This RFP may result in a single award or a multiple

award to contract with the County for the emergency department, express care and urgent care services. If an award is made, it is the County’s intent to award an agreement for up to a five (5) year term, with an option for the County to renew for an additional five (5) one-year term renewals, unless terminated earlier or otherwise amended. The Offeror and the County will agree upon a scope of services outlining the rights and responsibilities of each party through a contract.
