Uniforms can be provided for any NMDOT personnel\nidentified by NMDOT Management as a requirement of their job duties.\nTerm:\nThe term of this agreement shall be for one (1) year from date of award with the option to extend for a period(s) of three (3)\nadditional years, on a year-to-year basis, by mutual agreement of all parties and approval of the New Mexico State Purchasing\nAgent at the same price, terms and conditions. This agreement shall not exceed four (4) years.\nMultiple Awards:\nPursuant to the Procurement Code, Sections 13-1-153 and 13-1-154, the State reserves the right to issue multiple awards to obt\nthe items and services listed. Multiple awards are recommended to ensure availability and timely delivery. Awards will be mad\nmeet the best interests of the State of New Mexico.\nQuantities:\nQuantities on any item may be increased or decreased at any time at the request of the using agency. Quantities listed are\nestimated for bidding purposes only. The NMDOT reserves the right to order more or less as required to meet Department ne\u00ab\nBilling:\nVendor shall submit for payment on a monthly basis, billings and/or statements for actual services provided. Billing/stateme\u0131\nshall reference all invoice numbers which are covered by the billing/statement. Clear and proper billing procedures shall be t]\nresponsibility of the vendor.\nNon-Payment:\nVendor(s) will not be paid for items not delivered.\nTax Note:\nPrice shall not include state gross receipts tax or local option tax(es). Such tax shall be added at time of invoicing using currei\nrates and shown as a separate item to be paid by user.\nMinimum Requirements:\nUniforms & Linen:\nAt the inception of this Price Agreement, all uniforms and linen provided shall be new and of industrial quality material.\nNew employees eligible for uniforms and/or linens may request these items upon receiving written approval from their NMDC\nDistrict Engineer, area administrator, or designee. Vendor(s) shall be provided a written request prior to providing uniform\nand/or linens.\nFittings:\nAI NMDOT employees receiving uniforms from an awarded vendor(s) shall be properly measured and fitted by vendor prior\nissuance of uniforms.\nSize:\nSizes to be offered under this Price Agreement: X-Small through Extended sizes.\n" "[ssuance:\n\nEach NMDOT employee\u2019s first name will be identified on the inside of the shirt and jacket collar and the inside of the pants wai\nband.\n\nCleaning:\n\nFor each NMDOT employee uniform submitted for cleaning, employee will receive that exact amount in return.\n\nRepairs:\n\nThe vendor shall on a monthly basis, or upon written request from the employee, inspect uniforms and/or linen for tears, loose\nbuttons, loose seams and/or any other needed repairs. All repairs shall be done promptly and at no additional cost to the NMD\nIt is the employee\u2019s responsibility to rotate the use of uniforms to avoid excessive wear and tear or damage.\n\nReplacements:\n\nReplacement of uniforms and/or linen may be requested at any time during the life of this Price Agreement as long as the NMD\nDistrict Engineer, area administrator, or designee provides a written request. Requests of this type shall be for replacement of\nuniforms and/or linen which are torn, worn and/or no longer useable. The term worn shall be defined as uniforms that are fray\nthreadbare, old, ragged, deteriorated, torn and/or patched.\n\nReturns:\n\nPrior to issuance of a purchase order from the agency, the vendor will provide a written definition of what constitutes normal w\nand tear; abusive wear differentiates.\n\nIf an NMDOT employee leaves place of employment, all uniforms shall be returned within a two (2) week period.
