OMSP This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, and is being conducted according to FAR Part 13, Simplified Acquisition Procedures. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Eastern Business Service Center (EBSC) is issuing 1232SC18Q0018OMSP as a Request for quotations and incorporates provisions and clauses that are in effect through FAC 2005-99.  The National Center Agricultural Utilization Research (NCAUR) Peoria, IL has a requirement/need for Occupational Medical Surveillance Services. The award will be for the period starting approximately September 15, 2018 through September 14, 2023 for an IDIQ Service type contract.  The Details and specifications for the work shall be per the attached SOW and schedule B.The North American Industry Classification System (NAICS) code is 621511; small business size standard is
$32.5 Million.  DELIVERY TO: USDA-ARS-NCAUR, Peoria, IL.NOTE: ALL OFFERORS SHALL BE REGISTERED IN THE System for Award Management (SAMS) DATABASE IN ORDER TO RECEIVE AN AWARDDOCUMENTS TO BE INCLUDED IN QUOTE:  In order to be considered for award Offerors shall provide 1) a Completed Schedule B and Quote on company letterhead detailing the item description, total price and valid for at least 60 days after receipt of quote; 2) at least 3 to 5 references. (Government references preferred, with Contact Information). 3) a completed copy of the provisions FAR 52-212.3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS-COMMERCIAL ITEMS.REJECTION OF QUOTATION:   Failure to demonstrate compliance will be cause to reject the quote without further discussions. All responsible sources may submit an offer which will be considered.This procurement requires the use of biobased products to the extent that such products are reasonably available, meet agency or relevant industry performance standards, and are reasonably priced. Where available, these products should first be acquired from among qualified products that fall under the umbrella of items designated through the Federal Biobased Products Preferred Procurement Program (FB4P).Furnish Quotes by mail, or email to Mark O.
