This is a notice of intent, not a request for a quotation. A solicitation document will not be issued and quotations will not be requested.The National Institute of Health (NIH), Clinical Center (CC) intends to award a fixed price purchase order to University of Maryland at Baltimore, 725 West Lombard Street, Baltimore, MD 21201. The goal is to develop and apply a model that would provide the opportunity for early identification for those at risk for Opiod Use Disorder (OUD) and loss-to-follow-up, while guiding strategic interventions to prevent non-retention in care and reduce HIV transmission; an aim which align with the US DHHS’ goal of reducing incident HIV infections by 75% in the next 5 years.The Institute of Human Virology at the University of Maryland, Baltimore is the only known institution that has both the ready access to this HIV patient population and the relationship with an IT entity to implement this unique project.  This project address two critical elements of high
priority for achieving a 75% and 90% reduction in HIV transmission over the next 5 and 10 years, respectively.  This will require the development of a predictive algorithm which identifies those at high risk for OUD and allows for interventional strategies. Second, it will identify predictors of lack of retention of care in people with HIV (PWH) in urban clinic settings, which can be adapted to other similar settings.   The University of Maryland, Baltimore in its collaboration with an IT partner, Stellar, has developed a novel algorithm to review large sets of patient data retrospectively, without compromising patient privacy, in order to rapidly identify patients who are at risk for many comorbidities. Secondly, the University of Maryland, Baltimore has access to these data using the existing community-based clinical partnerships and embedded HIV clinics in Washington DC and Baltimore that that they run as part of their clinical program. The Baltimore and Washington regions have been hard hit by the HIV epidemic, with HIV rates of 2.9 and 2.3, respectively, and lifetime risk of HIV acquisition of 1 in 49 and 1 in 13, respectively, the highest in the United States. In parallel, Washington DC and Maryland claim the 4th and 5th highest rates of opioid overdose death nationally, and are the areas with the largest increase in overdose deaths nationally.  As such, the region represents a critical crossroads of the HIV and SUD epidemics, and is the ideal location to conduct a study targeting PWH. This acquisition is being conducted under the authority of the FAR Subpart 12 in conjunction with the policies and procedures for the competition requirements in FAR Part 6.  Interested concerns capable of providing the same or similar products or services as described in this notice may submit a capability statement outlining their capabilities.
