Sources Sought Information Request Form  Project Name: Child and Youth Services, Oregon Army National Guard Soldier Member Family Support Brief Description: The Oregon Army National Guard requires commercially available professional childcare services and teen youth activities for multiple Yellow Ribbon events throughout Oregon. This will be an indefinite delivery indefinite quantity (IDIQ) contract.  Each event takes place on a single day and the hours of service are 6:30 AM – 6:30 PM for the scheduled day. The contractor shall at all times maintain an adequate work force for the uninterrupted performance of all tasks defined within the SOW. Events are generally held in Oregon metropolitan areas such as Bend, Portland, Medford, Salem, and Springfield. The contractor will provide a customized age appropriate theme-based schedule of childcare programs, curriculum, and activities. The contractor will bring a sufficient amount of age appropriate toys, games, books, and arts and crafts
supplies to keep children, youth, and teens occupied and engaged for a full day in each age group room.  All childcare providers at the event who are employees of the contracted childcare company must meet the requirements under section 20351 of Title 34, United States Code. In accordance with section 20351 of Title 34, United States Code, each agency that hires (or contracts for hire), individuals involved with the provision to children under the age of 18 of child care services shall assure that all existing and newly hired employees undergo a criminal history background check. A background check shall be based on a set of the employee’s fingerprints obtained by a law enforcement officer and conducted through the Federal Bureau of Investigation and through the State criminal history repositories of all States that an employee lists as current and former residences.NAICS Code: 624110 – Child and Youth ServicesInterested firms please respond with this information form and the following requested information.
