Bid shall include, at a minimum, the following information in the order requested: 1. Letter of Transmittal. To include a statement by the respondent accepting all terms, conditions, and requirements contained in the RFB. The firms name and a contact person, along with their mailing address, telephone number, fax number, email address and website address, shall also be provided. 2. Company Background. A description of the age, history, ownership and mission of the firm. 3. Relevant Experience. A detailed description demonstrating significant experience providing complete solutions to other public agencies. Indicate how long your firm has been offering these services. If applicable, also include specific examples of how your firm has successfully customized similar solutions to serve the unique needs of a regional municipal purchasing cooperative. 4. Understanding/Approach to Provide Requested Services. A complete description of the work to be performed and what a complete service will

look like; a detailed description of your firms Compliance System, including if it is web-based, end-user system requirements, available modules, interfacing capabilities, the robustness of the system and available customization options; a description of the training program offered, including whether training is provided on-site, via the web, or both and if written reference materials are provided; 6. Insurance. All respondents are required to submit, with their responses, a copy of the cover sheet from their insurance policy(ies) (or an equivalent piece of documentation) which demonstrates the firms current coverage and limits for General Liability, Automobile Liability, Professional Liability and Workers Compensation Insurance as appropriate for the project work required herein. 7. Fee Proposal. All-inclusive fees for the performance of services outlined in Section IV above as well as the attached Scope of Services.
