Please read the Details section for information on how to bid on this opportunity Details Human Services Agency (HSA) Funding Opportunity Request for Proposals (RFP) #1064 Isolation Prevention Services for: Social Isolation Prevention for LGBTQ+ Older Adults and Adults with Disabilities, Animal Bonding Services for LGBTQ+ Older Adults and Adults with Disabilities, Care Navigation and Peer Support The San Francisco Human Services Agency’s Department of Disability and Aging Services (DAS) announces its intent to seek proposals from nonprofit organizations interested in contracting to provide the following three programs: 1. Social Isolation Prevention Services for LGBTQ+ Older Adults and People with Disabilities to provide care navigation, peer support and supportive programming to address the emotional, behavioral, health and social isolation challenges faced by LGBTQ+ older adults and adults with disabilities who are isolated. 2. Animal Bonding Services for Isolated LGBTQ+ Older

Adults and Adults with Disabilities to provide care navigation, peer support, supportive programming and animal bonding services to address the emotional, behavioral, health and social isolation challenges faced by LGBTQ+ older adults and adults with disabilities who are isolated and who have pets. 3. Care Navigation and Peer Support: to provide a care navigation and peer support program focused on serving individuals who fall within the identified target populations, listed in Section G of this RFP, to help mitigate barriers and create alternative pathways to reach older adults and adults with disabilities who may be reluctant to seek DAS services, find accessing services too complex, or may be unaware of their existence. A care-navigation and peer support program will facilitate service connections, provide support for consumers to further engage with available resources, and promote stability in the community. Respondents can apply for one, two or all three grants. For grantees applying for more than one grant, separate proposals are required. Applicants may elect to apply for this grant as a lead agency with collaborative partnership(s). Collaboration that creates a greater impact in service delivery through the sharing of skills, knowledge, and resources is encouraged. Applicants must clearly identify roles of all partnerships named in the proposal including lead agency.
