This is an ALL OR NONE bid. Note there are two (2) Lots for this solicitation. Bidders must bid on ALL Line Items in each Lot to be considered for an award. DO NOT include any sales taxes or government-imposed fees as County will calculate these at the time of the award. Bid prices should account for freight and/or delivery as all products are to be shipped FOB Destination, Freight Prepaid and Allowed. Bidders are to complete the Bid Response Form spreadsheet in its entirety and upload the file to Periscope

Bidders must enter the offered alternate product(s) on the Bid Response Form. County will be the sole judge if offered alternate product(s) is an approved equal and County's determination shall be final. Upon award, the vendor should contact the Public Health Department for the final format/layout of names/logos. Added on Apr 30, 2024:Addendum 1 is posted.
