The bidder is hereby noticed that the bidder will be required to gain DBE participation in the execution of the work to be done by this contract. The DBE goal is listed on the Notice to Bidders. If the Bidder cannot meet the specified goal, the Bidder shall submit a completed "DBE Information - Good Faith Efforts, Exhibit 15-H" form to receive consideration for award of this contract. The Bidder shall submit applicable list of subcontractors

and DBE information with the bid proposal. Failure to submit this information with the bid proposal may result in rejection of the bid. To satisfy Federal DBE regulations, Section 26.11 Part 26 Title 49 CFR, the agency requires information from all firms submitting bids, sub-bids, and quotes to develop a bidders list. Therefore, the Bidder shall: 1. Distribute copies of the “Construction Industry Survey” form, copy attached to the Proposal, to all subcontractors, suppliers and truckers who submit project sub-bids or quotes; and request them to complete the form and return them to the Bidder prior to bid submittal, and 2. Complete a copy of the “Construction Industry Survey” form and preferably submit it along with their sub-bidders forms in their project bid to the Agency; or at a minimum, submit all “Construction Industry Survey” forms within two (2) workings days of the bid opening.
