Include the physical location, along with a list of satellite offices and affiliates. Provide a brief company history. Include the date your agency or business was organized or created. Provide a list of current and former clients for whom the Contractor has worked with contracts similar in size and scope to the services outlined in this RFP. 1.2 (ME) Contractor must have three (3) years’ experience within the last five (5) years, providing large scale website development and maintenance, marketing, advertising, communication and promotional needs. List your current agency qualifications for meeting this requirement. Understanding of the Idaho State Board of Education Programs and Services: Demonstrate your agency's understanding of the Idaho State Board of Education’s strategic plan and education initiatives and efforts to address those needs. Understanding of the Next Steps Idaho website: Demonstrate your agency's understanding of the Next Steps Idaho website including audience segments, stakeholder partnerships as well as goals and initiatives. 1.3 Please acknowledge the following: Disclosure of Abortion Related Matters. The State is subject to the No Public Funds for Abortion Act, Idaho Code title 18, chapter 87 (the “Act”) and State employees who intentionally violate the provisions of the Act are subject to criminal prosecution. This provision is included in the Solicitation to aid in compliance with the Act. The State requests that Offeror disclose, unless Offeror is within one of the exemptions provided in the Act, if it or an affiliate is or becomes, during the term of the Contract (if Offeror is the awarded Contractor), an abortion provider and if it will use State facilities or public funds to provide, perform, participate in, promote or induce, assist, counsel in favor, refer or train a person for an abortion related activity. Please refer to the Act for definitions of the terms used in this section. 1.4 On April 26, 2021, Governor Little signed Senate Bill 1086. The bill creates a new section of code requiring all public entities in Idaho to insert a certification regarding boycotts of Israel in contracts and renewals of contracts that are effective on or after July 1, 2021. Offeror must agree to the following certification: failure to do so will result in the Offeror’s Bid/Proposal being deemed non-responsive. Do you certify as follows? Certification Concerning Boycott of Israel. Pursuant to Idaho Code section 67-2346, if payments under the Contract exceed one hundred thousand dollars ($100,000) and Contractor employs ten (10) or more persons, Contractor certifies that it is not currently engaged in, and will not for the duration of the Contract engage in, a boycott of goods or services from Israel or territories under its control. The terms in this section defined in Idaho Code section 67-2346 shall have the meaning defined therein. Group 2: Organization & Staffing Instructions: 2.1 (ME) Project Lead: Identify the person who will be the dedicated Project Lead if Contractor is awarded a contract. Provide a description of the proposed Project Lead’s experience and qualifications. Attach the individual’s resume and samples of their work. 2.2 (ME) Key Personnel and Qualifications Provide a list of key management, customer service and other roles to be used in the fulfillment of this Contract (in addition to the Project Lead). Provide role descriptions, including requisite qualifications and experience of the person(s)/role(s) identified, as well as an explanation of how the person in that role will contribute to the project. Attach the individual’s resume and samples of their work. Your response should demonstrate the extent to which you have the expertise to accomplish the Scope of Work. 2.3 (ME) Organizational Chart Provide a copy of your organizational chart, including detail of any relationships with parent and subsidiary organizations. 08 October 202108 October 202108 October 2021 2.4 Describe the extent to which subcontractors and outside service providers will be used to comply with contract requirements. Include each position providing service and provide a detailed description of how the subcontractors are anticipated to be involved under the contract. Include a description of how the Contractor will ensure that all subcontractors and their employees will meet all Business and Scope of Work requirements. All services provided must be limited to the services outlined in the Scope of Work and awarded under this contract, regardless of being provided by the Contractor, or by a subcontractor, and must be billed at the contract rates outlined in the Cost Proposal. Pass through costs for any products, services or subcontractors outside of the Scope of Work are not allowed. If you do not intend to utilize subcontractor(s), provide a statement to that effect.
