All equipment shall be provided by the vendor. Work Required: Provide any and all necessary materials, equipment and labor to take up, remove and dispose of current flooring and install and finish all aspects of new flooring. Removal of any refuse pertaining to delivery, un-packaging and installation within the scope of this work. Required Bonding/Insurance: Bid Bond (5%), Payment Bond (100%), and Performance bond (100%). Workers Compensation and General Liability Insurance are required as specified herein. Employee Status: Completed form is required if awarded the bid. Take up, remove and dispose of existing Council Chambers Flooring. 2. Provide and install Shaw All Access 24x 24 Sterling 34557 Lava Walk Off Carpet Tile in Council Chambers with Shaw N5100 Adhesive 3. Provide and install Rubber Stairnose-Black-Carper Tile Top. 4. After hours work required. All bids will be assumed to include the items listed below or elsewhere herein. Bidder agrees to perform all work and comply with all requirements as outlined in the bid documents and specifications. Local bidder preference per Cedar City Corporation Purchasing Policy. Prices shall include shipping, set-up and installation pertaining to this bid and the removal of any refuse pertaining to this bid. After hours work required. 1 Lump Sum 4. Contractor is required to provide security checks for all personnel assigned to work under this contract. Security 1 Lump Sum 7 checks will be coordinated through the Cedar City Police Department. The following information must be provided to the City Representative no less than 14 days prior to start of work: full legal name, social security number, Utah drivers license or identification number, birth date and current valid address TOTAL BID: Payment shall be disbursed in accordance with Cedar Citys accounts payable schedule following completion of the project and acceptance by Cedar City. Award of the bid will be subject to Cedar Citys Local Bidder Preference Policy as detailed in Cedar Citys Purchasing Procedures. Cedar City reserves the right to evaluate and award the bid to the Vendor who provides the bid that is deemed most beneficial to the City. Cedar City reserves the right to reject any and all bids. A bid bond in the amount of 5% of the total bid amount is required to be submitted with the bid. Consultant shall furnish CITY with the certificate provided in the Contract Documents covering this insurance. A. Workers Compensation: 1. State: Statutory 2. The retroactive date of the policy must be no later than the date of the Agreement. 6. If policies are written for split limits, limits shall be equal for bodily injury and property damage liability. 7. Cedar City Corporation shall be named as the Certificate Holder on the Certificate of Liability Insurance form. A signed endorsement from the insurance company stating Cedar City Corporation is named as an additional insured shall also be attached to the Certificate of Liability Insurance form. 9 CERTIFICATE OF INSURANCE Issue Date: This certificate is issued as a matter of information only and confers no rights upon the certificate holder.
