Create a coordinated of referral system to direct individuals to available resources with the goal of providing, through existing resources, a continuum of care of treatment and recovery services for OUD and any co-occurring SUD/MH conditions, including supportive housing, peer support services, counseling, community navigators, case management, and connections to community-based services.
Recovery Housing provides a safe, clean and sober environment for adults with Substance Use or co-occurring disorders. Recovery Housing will be available to ORF clients who are actively engaged in substance use treatment services and receive a recommendation for recovery housing.
The HFA model has been identified as the best primary home visiting model to support families with trauma, mental health and substance use disorders, as often they exist as co-occurring disorders. Parents that are impacted by a substance use disorder and mental health conditions often have high positive scores during the Healthy Families eligibility screening process.
Services must be based on the Recovery Model of values and principles for adults with or without co-occurring substance abuse disorders. Services will be provided for a maximum of 16 residential beds, 24 hours a day, 365 days a year (24/7).
